Nominate a Clinician of the Year!

This award recognizes the many contributions of clinicians using the Axxess CARE platform to deliver quality healthcare in the home. Nomination submissions are due by midnight EST on January 31, 2023

Award

A certificate signed by Axxess Founder and CEO John Olajide and $1,000 will be awarded to one RN, one LVN or LPN, one PT and one PTA among the 6,000 clinicians in the Axxess CARE network.

  • Consistent, standards-based high-quality care delivery in the home that improves patient outcomes
  • A robust understanding of industry regulations and best practices displayed in his or her clinical practices
  • Clear and careful communication with patients and their families that ensures continuity of care
  • A superior level of professionalism in the field and when working with organizations
  • Strength in completing thorough and compliant documentation in a timely manner

  • Three finalists per category will be named on February 10 by the industry expert selection committee based on submitted nominations. Voting will be open till February 24 at midnight EST.

  • The votes for finalists will be counted, and winners will be announced on March 20 on the Axxess LinkedIn page. Winners will also be contacted by phone.
